Sonata creates a comprehensive healthcare experience by providing complete palliative care where you live. This can be your private home, assisted living community or nursing home. Our hospice team helps patients and families make informed decisions, teaches the necessary skills for “hands-on” care, and guides families through the challenges associated with end of life care.
